A

Accidental Death & Dismemberment (AD&D)....... An amount paid to your beneficiary in the event of your accidental death. This is paid in addition to the Group Life Benefit. The Dismemberment Benefit is paid to you in the event of loss, or, loss of use of limb, sight etc.
Account Number................... A unique billing number assigned to each individual organization within the new HAL system.
AD&D................................. Accidental Death & Dismemberment
Annual Earnings................... Annual salary or annualized (base) earnings of casual, part-time or contract employee status. Annualized earnings are used as a multiplier of group life.

Global Assessment of Functioning (GAF)................. A numeric score (0 through 100) used by mental health clinicians and physicians to subjectively rate the social, occupational, and psychological functioning of adults, e.g., how well or adaptively one is meeting various problems-in-living. The scale is presented and described in the DSM-IV.
Great West Life Assurance Company (GWL)................... An agent of HBT that provides adjudicative services in respect of the Long Term Disability Plan administered by HBT.
Group.................................. Organization, single employer or unit of business.
Group Life............................ Group Life insurance product (can be single or multiples of annualized salary).
Group Life (GL).................... An amount paid to your beneficiary in the event of your death.

An IMC is most frequently requested when there are issues concerning Mental Health, Occupational Health and Addictions.
